The glass is (was, lol) about 1/4" thick.

Well then I'm quickly running out of ideas as to what caused it. You do know there is one spot on any piece of glass that is it's weak spot. It varies from piece to piece but each piece has one. Perhaps some tree rat jumped on the table at just the exact point and with the exact amount of pressure and broke it?

Could be, lol. I had that dang squirrel problem last winter. I've plugged the hole they were using, so maybe they're paying me back now, lol.
